2. Emergency Card
3. Health Information Form
4. TB Screening Form ( for parents only if you wish to volunteer )
   Note: If you completed the TB form in 2010 - 2011, you will not need to redo the test. It is good for 2 years. TB tests will be administered at CCNS on the Parent Volunteer Orientation Days in September.
5. Sworn Disclosure Statement ( for parents only if you wish to volunteer )
   Note: If you completed the Sworn Disclosure Statement in 2010 - 2011, you will not need to complete it this year. It is good for 2 years.

9. Infection Control Policy ( for parents only if you wish to volunteer )

We will also need to see your child's birth certificate or passport. Please bring one of these documents with you on the first day of school.
