School Application Form 2012- 2013
Please complete and return this form with your deposit to our school.


Permission To Photograph
There may be occasions during the school year that our students are videotaped for TV or have their picture taken to be posted on the CCNS website or used in CCNS printed materials (brochures, etc.).


Field Trip Permission
This form gives permission for your child to attend any field trips that his/her class will take as long as he/she is a student at Canterbury Community Nursery School.


Health Form
State law (Ref. Code of Virginia § 22.1-270) requires that your child is immunized and receives a comprehensive physical examination before entering public kindergarten or elementary school. The parent or guardian completes this page (Part I) of the form. The Medical Provider completes Part II and Part III of the form. This form must be completed no longer than one year before your child’s entry into school.


Carpool Letter
We’d like to help families match up with other families that would like to visit the carpooling option. Please fill out the questionnaire and submit to the CCNS office. We will compile the information and have it available for those who wish to carpool.


Infection Control Policy
In a child care setting, children come into contact with groups of other children outside their families. It is in this situation that the illness of one child can spread rapidly through the group to other children and staff members if stringent measures to prevent this spread are not taken.

Early Morning Drop-Off Registration
Please complete and return this form if you would like to use our Early Morning Drop-off program.


Afternoon Enrichment Club Registration
Please complete this form if you would like your child to participate in our Afternoon Enrichment Club.
