In an effort to ease the back-to-school paperwork that everyone seems to be inundated with in September, we are making available online the necessary forms for you to fill out for your child. Checklist for Returning Families

Emergency Card
Health Information Form (must be updated annually)
TB Screening Form (for parents...only if you wish to volunteer)
Note: If you completed the TB form in 2008 - 2009, you will not need to redo the test. It is good for 2 years. TB tests will be administered at CCNS on the Parent Volunteer Orientation Days in September.
Sworn Disclosure Statement (for parents...only if you wish to volunteer)
Note: If you completed the Sworn Disclosure Statement in 2008 - 2009, you will not need to complete it this year. It is good for 2 years.
